Registered Nurse Case Manager

We are seeking a skilled and experienced Registered Nurse Case Manager to join our team at St. David's North Austin Medical Center. As a Registered Nurse Case Manager, you will play a vital role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, ensuring appropriate resource management, and developing safe and effective discharge plans in collaboration with the multidisciplinary team.
Responsibilities:- Coordinate patient care activities to promote quality outcomes and patient throughput
- Develop and implement comprehensive care plans that address the psychosocial, medical, and discharge needs of patients and their families
- Collaborate with the multidisciplinary team to ensure seamless transitions of care and effective communication
- Identify patients at risk for adverse outcomes and develop strategies to mitigate these risks
- Partner with Social Services to ensure post-acute medical needs and level of care are appropriate
- 2+ years' experience in case management or 3+ years' experience in clinical nursing
- Associate Degree in Nursing or Nursing Diploma
- Bachelor's Degree in Nursing - preferred
