Registered Nurse Case Manager

We are seeking a highly skilled Registered Nurse Case Manager to join our team at St. David's North Austin Medical Center. As a Registered Nurse Case Manager, you will play a critical role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, and ensuring appropriate resource management.
Responsibilities- Provide case management services for inpatient and observation patients, identifying those at risk for adverse outcomes during transition from one level of care to another.
- Perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as resources available to them.
- Reassess patients' clinical conditions as needed, considering readmission status and developing strategies to mitigate risks.
- Coordinate plans of care and drive discharge plans by collaborating with multidisciplinary healthcare teams and patients' physicians.
- Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
- 2+ years' experience in case management or 3+ years' experience in clinical nursing.
- InterQual experience preferred.
- Associate Degree in Nursing or Nursing Diploma.
- Bachelor's Degree in Nursing preferred.
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
- Certification in Case Management preferred.
