Registered Nurse Case Manager

We are seeking a highly skilled Registered Nurse Case Manager to join our team at St. David's Medical Center. As a key member of our healthcare team, you will play a vital role in ensuring that our patients receive high-quality, efficient care.
About the Role:- Coordinate patient care plans to promote quality outcomes and patient throughput.
- Collaborate with multidisciplinary teams to develop safe and effective discharge plans.
- Identify patients at risk for adverse outcomes during transitions of care.
- Perform comprehensive assessments of patients' psychosocial, medical, and discharge needs.
- Develop strategies to mitigate readmission risks and educate patients on accessing healthcare resources.
- 2+ years' experience in case management or 3+ years' experience in clinical nursing.
- Associate Degree in Nursing or Nursing Diploma.
- Bachelor's Degree in Nursing preferred.
St. David's Medical Center is a 300+ bed facility located in central Austin, Texas.
