Job Title: Clinical Nurse Case Manager

Central Health is seeking a highly skilled Clinical Nurse Case Manager to join our team. As a key member of our care coordination team, you will play a vital role in ensuring the highest quality care for our clients.

Job Summary:

The Clinical Nurse Case Manager will work under the general direction of the Associate Director of Case Management to identify, screen, track, monitor, and coordinate care for clients. This role requires a strong clinical background, excellent communication skills, and the ability to work effectively in a dynamic team environment.

Key Responsibilities:
  • Collaborate with patients, families, and care teams to assess medical needs and develop comprehensive care plans.
  • Utilize clinical skills to review and monitor utilization of healthcare services, ensuring high-quality, cost-effective care.
  • Work in conjunction with multidisciplinary teams to provide case management and navigation services.
  • Coordinate care and communication between multiple providers, medical, nursing, social, and behavioral health.
  • Participate in continuous quality improvement projects to enhance patient care and service delivery.
Requirements:
  • Graduation from an accredited School of Nursing.
  • Minimum three years of clinical nursing experience in a hospital or clinic setting.
  • Current unrestricted RN license to practice nursing in the State of Texas.
  • Basic Life Support (BLS) certification.

This is an onsite position, and only candidates who live or will live in the Austin area will be considered for this role.
