Key Responsibilities

  • Lead the coordination and ensure the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with current Federal and State Regulations.
  • Oversee and document the management of Medicare and Managed Care residents in collaboration with facility team members.
  • Deliver innovative and responsible healthcare by developing and implementing new ideas and processes to enhance systems and achieve outstanding results.

Qualifications

  • Current, unencumbered state license to practice as an RN or LPN is required.
  • Preferred experience in Skilled Nursing or Rehabilitation facilities.
  • Minimum of six months of experience as a MDS Coordinator.
  • RAC-CT or RNAC certification is preferred.
  • Must demonstrate qualifications, compassion, and a commitment to excellence in performance.
