Key Responsibilities:

  • Supervise and actively engage in the execution of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with applicable Federal and State Regulations.
  • Oversee and document the management of Medicare and Managed Care residents in collaboration with facility staff.
  • Deliver innovative and responsible healthcare by developing and implementing new strategies and concepts that enhance systems and processes for optimal outcomes.

Qualifications:

  • Possession of a current, unencumbered, active state license to practice as an RN or LPN.
  • Preferred experience in Skilled Nursing or Rehabilitation facilities.
  • A minimum of six months of experience as a MDS Coordinator.
  • RAC-CT or RNAC certification is preferred.
  • Must demonstrate qualifications, compassion, and a commitment to excellence in performance.
