Job Description:

As a Hospice RN Case Manager, you will be responsible for providing high-quality end-of-life care, developing trusting relationships, and making a positive impact on patients' lives.

Duties and Responsibilities:

  • Evaluate patients' physical, psychosocial, and environmental needs, re-assessing as necessary.
  • Develop and implement personalized plans of care in collaboration with the patient, family, attending physician, and interdisciplinary team.
  • Maintain accurate clinical notes, documenting patient responses to hospice care.

Requirements:

  • One year of clinical experience as a Registered Nurse.
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.

Estimated Salary Range:$65,000 - $85,000 per annum, depending on location and experience.
