POSITION SUMMARY
Title: Community Resource Coordinator, LGH

Program: RISE

Classification: Non-Exempt/Full Time

KEY RESPONSIBILITIES

The following points outline the primary duties, responsibilities, and competencies deemed essential for fulfilling the core functions of this role. The organization reserves the right to modify specific job responsibilities as necessary.
  • Program Execution: Deliver immediate, client-focused assistance, resources, and referrals to individuals and families experiencing short-term crises, implementing tailored services, and advocating for client needs.
  • Information Management: Sustain a detailed database, financial records, and confidential documentation to effectively monitor community resources and client development.
  • Coaching Techniques and Application: Approach tasks with a client-centered perspective, employing diverse tools and strategies while continuously adapting and integrating new methods through ongoing professional growth. Facilitate training sessions organized by the agency for the community.
  • Collaborative Community Engagement: Proactively connect with various departments, agency programs, referral partners, and community initiatives to ensure a meaningful approach to community assistance.
  • Innovative Practices: Actively engage in meetings, contribute to program design and assessment, and identify new best practices for ongoing enhancement.
Perform additional job-related tasks as directed by supervisor.

EDUCATION AND EXPERIENCE REQUIREMENTS

Essential Qualifications:
  • Associate's degree in human or social services.
  • Clearances for Child Abuse, FBI, and PA Criminal Background prior to employment commencement.
  • In accordance with health policies, this position requires pre-employment TB testing and proof of immunization for two MMRs, Chickenpox, and the Influenza vaccine during the flu season.
  • Compliance with COVID-19 vaccination policies is mandatory for this role. Exemptions may be requested for medical or religious reasons.
  • As part of the onboarding process, lawful drug screening may be conducted in line with agency policies.
Additional Preferred Qualifications:
  • Bilingual candidates (Spanish/English) are preferred to ensure effective communication and support in clients' preferred languages.
  • A valid driver's license and access to a vehicle for travel throughout the service area are required.
  • Flexibility to work varied schedules, including occasional evenings and weekends based on client needs.
