Key Responsibilities

As a Crisis Intervention Case Specialist, you will:

  • Collaborate with Crisis Intervention team members to address the emotional, social, vocational, and housing needs of individuals during and after crisis situations.
  • Provide referral and case coordination services to help individuals achieve stability in their respective environments.
  • Utilize community resources effectively to assist individuals in need.
  • Participate in service planning by considering social factors relevant to the development of emergency and post-emergency service plans.
  • Prepare service plans and other necessary documentation to support case coordination goals.
  • Ensure the delivery of intensive post-crisis case management services that facilitate ongoing recovery.
  • Conduct supportive services through various methods, including in-person, in-home, telephone, or teletherapy, as appropriate.
  • Adhere to all regulations governing service provision, including maintaining accurate records.
  • Engage with consumers in a legal and ethical manner.
  • Perform additional related tasks as assigned.
