Crisis Case Manager Job Summary
AHRC NYC is seeking a skilled Crisis Case Manager to join our Crisis Intervention team in New York City. The successful candidate will work collaboratively with team members to support individuals in crisis, providing referral and case coordination services to achieve stability and well-being.
Key Responsibilities:
- Collaborate with Crisis Intervention team members to identify and address emotional, social, vocational, and housing needs.
- Provide referral and case coordination services to support individuals in achieving stability.
- Utilize community resources to assist individuals in crisis.
- Participate in planning for services, addressing social factors pertinent to emergency and post-emergency service plans.
- Prepare service plans and other plans as needed to substantiate case coordination plan goals.
- Deliver intensive, post-crisis case management services to support ongoing crisis recovery.
Requirements:
- Bachelor's degree in a Human Services discipline from an accredited university program.
- Work experience with people with developmental disabilities.
- Strong interpersonal, written, and verbal communication skills.
- Excellent skills in using MS Office applications and agency-specific database systems.
- Proven capacity to work productively in highly collaborative settings and coordinate needs of multiple stakeholders.
- Ability to make decisions independently and react to unanticipated events in a prudent and timely manner.
