LPN Residential Nurse

We are seeking a skilled LPN to join our team at AltaPointe Health. As a Residential LPN, you will play a vital role in providing high-quality care to our consumers. Responsibilities- Administer medications, observing the Seven Rights of medication administration and monitoring consumers for potential side effects of medications and report findings to the physician.
- Provide medication education for the consumer and family, including uses and potential side effects.
- Review MAR daily to ensure appropriate medication management which includes timely refills and any necessary arrangements with medical staff for new prescriptions.
- Identify psychiatric and medical needs of consumers and communicate effectively with the physicians to include taking off any orders and completing consults.
- Observe consumers behavior in groups and activities, interact with consumers and evaluate needs to ensure that proper information is reported from shift to shift.
- Interface with the multi-disciplinary team and actively and cooperatively participates in development of the interdisciplinary treatment plan.
- Monitor laboratory results and reports all abnormal findings to the physician.
- Demonstrate the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation, including providing emergency care as needed (CPR, MindSet, PRN medications).
- Conduct infection control training for consumers.
- Ongoing compliance with the Nurse Delegation Program, including recertification requirements, including taking MAS calls.
- Assist consumers with personal care issues (bathing, toileting, and other personal hygiene issues).
- Perform monthly MARs verifications and 24 hour chart checks.
- Conduct individual and group PHP trainings, activities, and education.
- Must be a current LPN in the state of Alabama.
- Must have a valid driver's license.
The LPN will work in a fast-paced residential setting, interacting with consumers, families, and multidisciplinary teams. The position requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
The LPN will be required to stand for extended periods, walk various distances, and lift up to 40-50 pounds. The position also requires the ability to work in a crisis situation and to provide emergency care as needed.
