About the Role:

We are seeking a skilled Social Service Coordinator to join our team. The successful candidate will be responsible for providing emotional and administrative support to clients in individual or group settings.

Key Responsibilities:

  • Client Support: Provide emotional and administrative support to clients, addressing their physical, emotional, and social needs.
  • Care Planning: Develop and implement individual care plans in collaboration with clients, families, and healthcare professionals.
  • Progress Monitoring: Monitor client progress, making adjustments to care plans as needed.
  • Communication: Communicate client updates to relevant parties, ensuring seamless coordination of care.
  • Referrals: Facilitate referrals to other healthcare professionals and programs as required.
  • Documentation: Maintain accurate and up-to-date client documentation.

Qualifications:

  • Experience: Previous experience in social work, counseling, or other related fields.
  • Skills: Compassionate and caring demeanor, ability to build rapport with clients, strong leadership qualities, and excellent written and verbal communication skills.

Salary: $70,000 - $90,000 per year
