Overview:
The Patient Assessment Coordinator plays a vital role in identifying patients for potential admission into skilled nursing and rehabilitation facilities. This position is essential for maintaining strong relationships with referral sources and ensuring a smooth admission process.
Key Responsibilities:
1. Conduct comprehensive clinical assessments to evaluate patient needs.
2. Serve as a liaison between referral sources and the admissions team, facilitating the transmission of clinical and benefit information.
3. Provide exceptional customer service to all stakeholders, ensuring a positive experience for patients and families.
Qualifications:
- Current and valid RN or LPN license in the State of Ohio, or a degree in Social Work, or other relevant medical credentials.
- A minimum of three years of treatment experience in a Long Term Care environment is preferred.
- Previous experience in clinical screening and knowledge of the local medical community is advantageous.
Skills Required:
- Strong communication skills, both verbal and written, with the ability to convey clinical information professionally.
- Excellent organizational and analytical abilities to prioritize tasks effectively.
