**Job Summary**

We are seeking a highly motivated and organized Patient Navigator to join our team at Monument Health Rapid City Hospital. As a Patient Navigator, you will be responsible for navigating patients and family members through the diagnosis, treatment, and follow-up of an episode of care related to an identified organizational clinical service program.

This position requires strong communication and interpersonal skills, as well as the ability to work effectively with multidisciplinary teams to achieve optimal outcomes for patients. Key Responsibilities:

  • Acts as a resource to interdisciplinary teams, patients, and families throughout the episode of care.
  • Supports patients and families by providing education, advocacy, addressing psychosocial needs, and identifying and addressing barriers to care.
  • Utilizes evaluative and outcomes data to facilitate the achievement of consistent clinical outcomes.
  • Organizes and completes a plan customized for each patient referred, including scheduling, supportive resources, and post-treatment resources.
