LPN Field Care Specialist

In My Home Hospice is seeking a skilled LPN to join our team as a full-time Palliative Home Healthcare Provider. This role involves working directly with patients and their families in the Phoenix Metro Area, primarily East Valley.
Key Responsibilities
The successful candidate will:
- Deliver high-quality palliative care to patients in their homes.
- Work collaboratively with the interdisciplinary team to coordinate care and address patient needs.
- Manage pain and symptoms, evaluating patient responses to treatments and medications.
The ideal candidate will possess excellent communication skills, a sympathetic attitude towards caring for hospice patients, and the ability to work independently.
Requirements
To be considered for this role, candidates must:
- Hold a current LPN/LVN license in Arizona.
- Have a minimum of 1 year of community health or hospice experience (training provided for the right candidate).
- Demonstrate good verbal and written communication skills, as well as organizational abilities.
- Possess a valid driver's license and reliable vehicle insurance.
