Job Summary: We are seeking a dynamic and talented Referral Management LPN PRN Specialist to join our team. As a key member of our referral management team, you will play a crucial role in reviewing assessment referrals and collaborating with RNs to determine the appropriate placement of patients.

Responsibilities:

  • Review assessment referrals and collaborate with RNs to assess patient needs.">
  • Assist with coordination of patient care and maintain accurate patient records.">
  • Ensure timely and efficient communication with stakeholders to ensure seamless patient flow.">

Requirements:

  • ">
  • Nursing Degree from Vocational/Technical School or Associates Degree in Nursing.">
  • Licensed LPN in the State of Arizona or compact license.">
  • Preferred 1-year related work experience.">
  • Current CPR certification, if required by customer hospital.">
