In My Home Hospice offers a rewarding opportunity for an LPN to join our team as a full-time Field Care Specialist. This role involves working directly with patients and their families in the Phoenix Metro Area, primarily East Valley.
Job Summary
The successful candidate will deliver high-quality care and support to patients and their families, working collaboratively with the interdisciplinary team to coordinate care and address patient needs.
- Providing nursing and palliative care to patients in their homes.
- Managing pain and symptoms, evaluating patient responses to treatments and medications.
The ideal candidate will possess excellent communication skills, a sympathetic attitude towards caring for hospice patients, and the ability to work independently.
Requirements
To be considered for this role, candidates must:
- Hold a current LPN/LVN license in Arizona.
- Have a minimum of 1 year of community health or hospice experience (training provided for the right candidate).
- Demonstrate good verbal and written communication skills, as well as organizational abilities.
- Possess a valid driver's license and reliable vehicle insurance.
