About the Role

We are seeking a dedicated Care Coordination Specialist to support individuals on their recovery journey.

The ideal candidate will have 2+ years of experience in behavioral health and a passion for delivering individualized care management services.

Key Responsibilities:
  • Develop and implement personalized recovery plans in collaboration with clients and their support systems.
  • Conduct assessments and facilitate access to resources and services that promote successful outcomes.
  • Maintain effective communication with clients, healthcare providers, and other stakeholders to ensure seamless coordination of care.

This role requires strong analytical and problem-solving skills, as well as the ability to work collaboratively in a fast-paced environment. The ideal candidate will be a team player with a patient-centered approach.

Qualifications:
  • Bachelor's degree in social work, psychology, or a related field is preferred.
  • Minimum of 2 years' experience in behavioral health or a related field.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • Proficiency in electronic medical records (EMRs) and other software applications relevant to the position.
