Hospice Field LPN Specialist

In My Home Hospice Care is seeking a full-time LPN to provide nursing and palliative care to assigned hospice patients in the Phoenix Metro Area, primarily in the East Valley.
About the RoleThe LPN will work under the direction of a Registered Nurse and be responsible for providing compassionate care to patients and their families. The ideal candidate will possess excellent communication skills and be able to work effectively as part of an interdisciplinary team.
Responsibilities- Provide nursing and palliative care to assigned hospice patients
- Participate in coordination of hospice services and report identified needs to the interdisciplinary group
- Evaluate patient response to treatments and medications, and provide appropriate pain and symptom management
- Perform other duties as assigned by the registered nurse
- Graduate of an accredited practical nurse or vocational nursing program
- Community health/hospice experience preferred but willingness to train the right candidate
- Currently licensed as an LPN/LVN in the State of Arizona
- Demonstrates good verbal and written communication, and organization skills
- Possesses and maintains current CPR certification
