**Responsibilities:**

* Provide medical care to hospice patients, including history and physical exams, diagnosis and treatment of medical problems, and education and counseling of individuals, families, and groups.
* Determine hospice eligibility and pain and symptom management, including face-to-face patient visits.
* Participate in the care of the hospice patient as a member of the interdisciplinary team.

**Qualifications:**

* Must be a graduate of an approved school and licensed as a nurse practitioner in the state in which they currently practice.
* Must have basic demonstrated technology skills, including operation of a mobile device.
* At least five years of nursing experience with a minimum of one year in hospice is preferred.

**Requirements:**

* Must possess a valid state driver's license and maintain automobile liability insurance as required by law.
* Must maintain dependable transportation in good working condition and be able to safely drive an automobile in all types of weather conditions.
* Must possess CPR certification for the healthcare provider.
