Hospice Nurse

About the Role:
We are seeking a skilled and compassionate RN to join our team as a Hospice Triage and Care Specialist. In this role, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities:
- Conduct thorough assessments of patients' needs and develop individualized care plans to maximize their comfort and quality of life.
- Provide skilled nursing visits, including wound care, medication management, and symptom control.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and regulatory requirements.
Requirements:
- Current CA RN license.
- At least 12 months experience in acute care, with 1-2 years preferred.
- Hospice experience a plus.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
