We are seeking a skilled and compassionate Hospice Triage Nurse to join our team at The Elizabeth Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care, assess patient needs, and develop individualized care plans to maximize patient comfort and quality of life.
Responsibilities:
- Provide skilled nursing visits, assessments, and care to patients across the lifespan, consistent with The Elizabeth Hospice protocols and standards.
- Collaborate with physicians, supervisors, and other healthcare professionals to ensure seamless patient care.
- Develop and implement individualized care plans to address patient needs and preferences.
- Provide education and support to patients, caregivers, and family members to promote self-care and independence.
- Document patient care and progress in a timely and accurate manner.
Requirements:
- Current CA RN license.
- At least 12 months experience in acute care, with 1-2 years preferred.
- Hospice experience preferred.
- Bi-lingual (Spanish) preferred.
- Excellent customer service skills.
- Computer skills including Word, Outlook, and Excel.
- Current California driver's license with proof of insurance required.
