Position Overview:

Cogir Senior Living is seeking a compassionate and experienced Resident Care Coordinator (LPN/LVN) to join our esteemed retirement community, Cogir of Mill Creek. In this pivotal role, you will serve as a vital link between residents, their families, and our dedicated staff.

The Resident Care Coordinator is responsible for the comprehensive implementation, delivery, and management of resident care within the community. You will assist the Health and Wellness Director in scheduling, staff management, and ensuring that effective systems are in place to provide high-quality assisted living services across all aspects of resident care. This role also involves addressing resident concerns and coordinating care with both the caregiver team and external partners.

Key Responsibilities:

  • Oversee, develop, and schedule care staff in accordance with company policies and standards.
  • Coordinate admissions with assistant administrators, including overseeing move-ins.
  • Maintain up-to-date resident records and documentation.
  • Collaborate with home health agencies on-site for care planning, adhering to community policies.
  • Act as a liaison with families to ensure that special needs, requests, and concerns are addressed.
  • Assist in organizing staff training and in-service schedules.
  • Supervise the medication room and orders, collaborating with Medication Aides and Caregivers.
  • Serve as the primary contact for all matters related to resident care.
  • Understand and comply with community care regulations as well as state-specific guidelines.

Qualifications:

  • A collaborative team player with a passion for serving the senior community.
  • 3-5 years of experience in geriatric nursing, including supervisory responsibilities.
  • Strong verbal and written communication skills.
  • High level of accuracy in all tasks and effective time management abilities.
  • Proactive approach, sound judgment, and integrity.
  • Current First Aid and CPR certification or the ability to obtain it.
