OVERVIEW
The Housing Support Specialists will deliver essential housing assistance services aimed at aiding individuals experiencing homelessness or those at risk of losing their homes in securing safe and permanent accommodations. The role involves conducting thorough housing searches within the private market and various subsidized housing initiatives, collaborating with landlords and community organizations to identify affordable housing solutions for those in need.
KEY RESPONSIBILITIES
- Offer housing search support to clients referred by healthcare organizations or medical practices.
- Facilitate referrals to supportive housing programs that include care management and rental assistance as required.
- Perform a comprehensive housing barriers assessment with each client, collaborating with them and the case management team to formulate and implement a rehousing strategy.
- Establish connections with local landlords to create placement opportunities for individuals facing disadvantages.
- Assist clients in navigating the subsidy application process, particularly for those on fixed incomes.
- Evaluate housing and support service needs to identify available short-term financial assistance programs aimed at rapid rehousing.
- Guide clients in gathering necessary documentation and completing applications for housing-related processes.
- Support clients in applying for financial assistance programs such as RAFT, ESG, or other transitional funding options as they become available.
- Assess and process requests for client assistance funds, including first, last, and security deposits, as well as essential items like furniture and linens.
- Evaluate and process applications for necessary home modifications to enhance accessibility.
- Assist clients in completing housing applications and negotiating lease agreements with landlords.
- Accompany clients during property viewings and help them prepare for application and interview processes.
- Collaborate with team members to provide initial information and referrals for community resources, including education, job training, and healthcare services.
- Participate actively as a member of the team, attending all required meetings and contributing to team objectives.
- Maintain comprehensive client records, including assessments, rehousing plans, and services utilized.
- Collect and input required demographic data into designated databases.
- Engage with clients through various communication methods, ensuring their needs are understood and addressed.
- Uphold the confidentiality of client, employee, and agency information in accordance with applicable laws and regulations.
- Ensure adherence to program, agency, and funder requirements, as well as organizational policies and procedures.
- Perform additional duties as assigned.
QUALIFICATIONS AND SKILLS
- A Bachelor's degree is preferred, along with a minimum of two years of relevant experience in social or housing services, particularly with low-income, minority, and disabled populations.
- A valid driver's license and reliable transportation are essential.
ORGANIZATIONAL STRUCTURE
- This position reports directly to the Program Manager.
- There are no direct reports associated with this role.
PHYSICAL DEMANDS
- Ability to sit or stand for extended periods.
- Proficiency in operating a computer and completing extensive documentation.
- Capability to lift up to 25 lbs.
- Ability to navigate multiple flights of stairs, as some housing units may be located on higher floors.
- Flexibility for remote work opportunities is available.
- Business casual attire is expected.
- Frequent travel within the local community to meet clients and explore housing options is required.
WORK ENVIRONMENT
This position involves direct or incidental contact with clients served by the organization in various programs funded or administered through relevant state offices. A successful background check is mandatory.
