Job Title: Hospice Care Manager Lead

Median Salary: $115,000 per year (based on national averages and location)

Responsibilities:
  • Approve requests for services, re-certifications, add-on orders, and changes to ensure patient care meets high standards.
  • Oversee scheduling of all patient services to optimize resource allocation and meet regulatory requirements.
  • Receive and approve patient re-certifications and discharges while ensuring timely updates to electronic health records.
  • Audit physician modified orders for accuracy, legibility, and content prior to forwarding them to the physician's office for signature.
  • Review and sign lab reports, notifying physicians of any clinical facts related to patients' conditions.
  • Conduct in-home supervisory visits to assess patient needs and ensure compliance with treatment plans.
Requirements:
  • Hold a current RN license in the state of PA from an accredited nursing school.
  • Possess at least one year of hospice experience or two years of overall nursing experience.
