POSITION SUMMARY

The Emergency Services Case Manager I will report directly to the Emergency Services Manager and will be responsible for delivering case management services, crisis intervention, and support for clients enrolled in emergency services programs. This role involves tracking client progress and connecting them with appropriate resources as needed.

KEY RESPONSIBILITIES
  • Uphold personal and professional integrity while ensuring clients are treated with respect.
  • Handle confidential information with care, adhering to privacy regulations.
  • Provide comprehensive case management aimed at helping clients achieve housing stability.
  • Establish rapport with clients and facilitate effective group discussions.
  • Maintain an updated directory of community resources and assist clients in accessing these services.
  • Document client interactions and maintain accurate case notes.
  • Ensure timely and accurate entries into the Homeless Management Information System (HMIS).
  • Organize client files and ensure all documentation is complete.
  • Communicate challenges and successes with supervisors regarding complex cases.
  • Support clients during their transition from homelessness to stable housing.
  • Collect contact information from clients upon their departure for follow-up purposes.
  • Close client files appropriately and ensure database updates are made for discharges.
  • Participate in collaborative meetings and network with other agencies as required.
  • Foster effective communication with clients and colleagues.
  • Engage in ministry activities as part of the organizational mission.
  • Perform additional duties as assigned.
QUALIFICATIONS
  • High School diploma or GED is required.
  • A minimum of one year of experience working with homeless populations is preferred.
  • Ability to exercise independent judgment within established guidelines.
  • Familiarity with mental health and substance abuse issues to facilitate appropriate referrals.
  • Knowledge of local community resources and programs available to clients.
  • Ability to build and maintain effective relationships with diverse populations.
  • Capability to respond calmly and effectively in emergency situations.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) is necessary.
  • Strong command of English for effective communication.
PHYSICAL DEMANDS
  • Ability to sit for extended periods and lift/carry objects weighing up to 25 pounds.
  • Must be able to see, hear, and speak to interact with staff and clients.
  • Ability to move around as necessary to fulfill job responsibilities.
