Position Overview:
As a Case Management Specialist, you will be responsible for creating comprehensive care plans in collaboration with patients, monitoring their ongoing health status, and ensuring continuity of care. Your role will involve reviewing patients' claims data, laboratory results, and prescriptions, while actively engaging with patients to empower them in managing their health and setting achievable treatment goals.
Key Responsibilities:
1. Conduct disease management for members with chronic conditions.
2. Perform initial assessments to evaluate patients' understanding of their conditions and identify educational needs.
3. Collaborate with patients to develop tailored disease management plans that align with their goals.
4. Monitor patient cases by tracking attendance at appointments, evaluating treatment responses, and advocating for necessary services.
5. Provide ongoing support and education to patients and caregivers, either in-person or via phone, until they transition back to their primary care provider.
6. Ensure timely follow-up for patients discharged from inpatient care within 7 days.
7. Communicate care plans effectively with physicians, patients, and care team members, remaining attentive to any changes in patient conditions.
8. Maintain accurate client records by documenting case notes and progress.
9. Ensure timely contact with members enrolled in the Disease Management program, adhering to documentation and follow-up protocols.
10. Assess patients' needs through intake assessments and coordinate therapeutic evaluations as necessary.
Required Competencies:

Certification Requirements:
Accredited Program (Required)
BLS Certification from recognized organizations
Licensed Practical Nurse (LPN) License - State of Florida
Additional Information:
Employment in this role is contingent upon successful completion of a Level II Criminal History record check.
Experience:
Minimum of one (1) year of experience as a Licensed Practical Nurse is required.
Working Conditions:
This position requires the ability to perform various physical tasks, including lifting, standing, and sitting for extended periods.
In addition, the role may involve exposure to biological hazards and requires adherence to safety protocols.
