We are seeking a highly skilled Patient Advocate Representative to join our team at Conifer Health Solutions. As a Patient Advocate Representative, you will play a critical role in ensuring that patients receive the best possible care and support throughout their healthcare journey.
Key Responsibilities- Conduct thorough interviews with patients and/or family members to assess their eligibility for various government and non-government programs.
- Record and maintain accurate documentation of all activities performed on patient accounts, both in-house and during the patient accounting cycle.
- Perform financial clearance functions, including collections, and cancel accounts that are not eligible for any programs.
- Develop and maintain strong relationships with patients, based on excellent communication skills, to ensure timely processing of government program eligibility.
- Conduct field visits to patient homes for skip tracing and assisting patients with documents.
- Notify hospital case management, social services, and admissions staff of case screening determinations and outcomes via verbal and written communication.
- High school diploma or equivalent required; minimum 2 years of work experience in social services or hospital admitting/related area preferred.
- Working familiarity with rules and regulations pertaining to federal, state, and county programs.
- Ability to work independently and prioritize multiple tasks with efficiency.
- Excellent oral and written communication skills, with a clear understanding of the English language.
- Bi-lingual preferred (Spanish).
This role requires the ability to sit and work at a computer terminal for extended periods, as well as the ability to walk through a hospital environment and visit patients at bedside. Some travel may be required.
