Job Title: Nurse Practitioner

TruHealth LLC is seeking an experienced Nurse Practitioner to join our team of healthcare professionals. As a key member of our team, you will be responsible for providing high-quality patient care to our residents in long-term care facilities.

Job Summary

We are looking for a skilled Nurse Practitioner with experience in geriatric care to join our team. The ideal candidate will have a strong background in primary care and internal medicine, with a focus on geriatrics. You will work closely with our team of healthcare professionals to provide comprehensive care to our residents, including assessing their medical and mental health needs, developing treatment plans, and monitoring their progress.

Responsibilities
  • Assess patients' medical and mental health needs and develop treatment plans to proactively manage their conditions and address barriers to care
  • Ensure compliance with local, state, and federal agencies related to clinical services provided
  • Prescribe medications and order lab work, diagnostic procedures, and consultations as needed
  • Monitor patients' compliance and response to treatment and modify plans as necessary
  • Work with RN case managers as part of an integrated care team
Requirements
  • FNP, AGNP, AHACNP, or PA license required
  • Degree from an accredited APRN or PA program
  • 3 years' experience in clinical nursing or rehab in geriatric populations
  • Electronic Health Records experience
  • Working knowledge of Microsoft applications, including Word, Outlook, and Excel
Preferred Qualifications
  • Primary care experience in geriatric medicine
  • 3 years' experience in long-term care
Education
  • Masters or better
