Position Overview:

As a Licensed Vocational Nurse (LVN) at Alhambra Healthcare & Wellness Center, you will operate under the guidance of a Registered Nurse, delivering essential nursing care and services to residents in a long-term care environment.

Qualifications:

  • Completion of an accredited nursing program or an approved LVN curriculum.
  • Current nursing license in good standing with the relevant State board.
  • Excellent communication and organizational abilities.
  • Capability to deliver high-quality patient care in alignment with established standards, policies, and procedures.
  • Proficient in identifying and reporting issues, demonstrating initiative and sound judgment in decision-making.
  • Thorough understanding of nursing principles and practices to uphold care quality standards for residents.
  • Willingness to collaborate with both professional and non-professional staff. Proficiency in English is required; bilingual in Chinese is preferred.
  • Valid CPR certification.

Key Responsibilities:

General Duties:

  • Deliver nursing care as directed by healthcare professionals, adhering to legal practice scopes and established care standards.
  • Administer services that support residents in achieving or maintaining their optimal physical, mental, and emotional well-being.
  • Provide clinical data and observations to assist in the nursing care plan.

Clinical Responsibilities:

  • Manage the admission, transfer, and discharge processes for residents according to facility policies.
  • Orient new residents and their families as needed.
  • Exchange nursing reports at the start and end of shifts, reviewing daily patient assignments with Nursing Assistants.
  • Ensure the unit is maintained in a clean and safe condition for resident comfort.
  • Conduct daily rounds to assess residents' physical and emotional conditions.
  • Collect and report significant clinical findings in accordance with policy.
  • Perform nursing skills under the supervision of a Registered Nurse.
  • Document care accurately and timely, including resident observations and responses.
  • Assist in the development and revision of resident care plans as necessary.
  • Receive and accurately transcribe physician orders.
  • Administer medications as prescribed, adhering to legal practice scopes.
  • Evaluate responses to treatments and report any adverse reactions.
  • Maintain secure and accurate records of narcotics and controlled substances.
  • Order medications and supplies as per facility protocols.
  • Coordinate diagnostic and therapeutic services as directed by physicians.
  • Notify physicians and guardians of significant changes in resident conditions.
  • Document incidents and unusual occurrences, initiating investigations as needed.
  • Communicate significant changes in resident conditions to the RN Supervisor.
  • Participate in various facility meetings as required.
  • Report any allegations of abuse or unusual occurrences in accordance with regulations.
  • Initiate emergency procedures and provide necessary treatments.
  • Engage in infection control programs to monitor communicable diseases.
  • Assist in the orientation of new staff members.
  • Supervise Certified Nursing Assistants (CNAs).

Consumer Service:

  • Present a professional demeanor through attire and communication.
  • Treat residents and their families with respect and dignity.
  • Address resident and family concerns, assisting in the resolution of grievances.

Additional Responsibilities:

  • Participate in the facility's Quality Assurance program and assist in addressing findings from health services.
  • Ensure compliance with health information protection regulations.
  • Attend in-service training and educational programs to maintain nursing competencies.
  • Perform other duties as assigned.
