About the Role:

The Hospice Social Worker position is a full-time opportunity with a comprehensive benefits package, including a 401(k) with match, paid holidays, paid time off, medical, dental, and vision insurance, short and long-term disability insurance, life insurance, continuing education funds, mileage reimbursement, and cell phone reimbursement.

Job Summary:

The Hospice Social Worker is responsible for providing psychosocial, emotional, and resource support to patients and families facing end-of-life issues. This role requires a strong clinical judgment and ability to work independently, with a focus on delivering high-quality care and services.

Key Responsibilities:

  • Completing admission and transfer processes with patients and families, including explaining hospice services and insurance benefits, and completing necessary paperwork.
  • Providing counseling and education on the psychological, emotional, and social aspects of death and dying.
  • Assisting patients and families in identifying and accessing resources to meet their environmental, financial, legal, or psychological needs.
  • Documenting patient and family interactions and activities in a timely and accurate manner.

Requirements:

  • A Master's degree in social work from a Council on Social Work Education-accredited program, with current Michigan Social Work licensure as a clinical social worker.
  • A valid Michigan driver's license and reliable transportation.
  • Sound clinical judgment and ability to work independently.
  • Knowledge of and/or experience with end-of-life care is preferred.
  • Excellent communication and charting skills.
