About the Role

We are seeking a skilled Admissions Registered Nurse to join our team at Hospice of Holland. As an Admissions Registered Nurse, you will play a critical role in ensuring that our patients receive the highest level of care and support during their time with us.

Key Responsibilities
  • Admissions Process: Determine if patients meet criteria for admission into our hospice program through visual contact, interviewing, and reviewing medical history.
  • Initial Assessment: Complete the initial assessment of the admissions process to address the immediate care needs of patients and families.
  • Education and Support: Provide education and support related to symptom management and end-of-life care for patients, families, and caregivers.
  • Communication: Communicate patient and family needs to assigned Registered Nurse Case Managers and on-call staff as necessary.
  • Consultation: Consult with physicians for appropriate orders and assist in obtaining necessary supplies, equipment, medications, and services.
Requirements
  • Education: Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan.
  • Experience: Two years of recent clinical experience preferred, but recent nursing school graduates are also encouraged to apply. Homecare or hospice experience desired.
  • Clinical Judgment: Sound clinical judgment and knowledge of pain symptom management with end-of-life care.
