Palliative Care Acute Care Nurse Practitioner Job Summary

Baylor Scott & White Health is seeking a highly skilled Palliative Care Acute Care Nurse Practitioner to join their team. The ideal candidate will have a minimum of 2 years of experience in direct palliative care as an APP and be bilingual in English and Spanish.

Key Responsibilities:
  • Provide comprehensive assessments, disease and treatment education, and symptom management to meet the needs of palliative care patients and their families.
  • Develop and implement comprehensive outcomes-based patient treatment plans based on patient assessments, evidence-based medicine, and professional practice guidelines.
  • Counsel patients and families regarding medical processes, management of the health illness/condition, medication, nutrition, and health promotion.
  • Prescribe, recommend, and/or employ medical drug therapies or other forms of related therapeutic treatments and procedures.
  • Maintain accurate and detailed records of patient treatment plans and outcomes.
Requirements:
  • Graduate of an accredited program.
  • 2 years of experience as an AGACNP, with ICU experience preferred.
  • Bilingual English/Spanish preferred.
  • Acute Care Nurse Practitioner (NP) certification.
  • Registered Nurse (RN) licensure.
  • Basic Life Support (BLS) certification within 30 days of hire or transfer.
