Job Summary

We are seeking a highly skilled and compassionate Social Service Professional to join our team at Axesspointe Community Health Center. As a Foster Care and Adoption Specialist, you will play a critical role in providing support and services to birth families, foster families, and adoptive families.

Key Responsibilities
  • Provide direct services to children, biological families, foster families, and adoptive families, including case management, counseling, and advocacy.
  • Develop and implement comprehensive assessments of individuals and families, and provide/secure appropriate services to address their needs.
  • Collaborate with agency staff, clients, and community partners to ensure seamless service delivery and achieve desired outcomes.
  • Maintain accurate and confidential case records and information, and prepare clear and concise reports and documents as required.
  • Participate in phone calls, home visits, court hearings, and other appointments/meetings as needed to ensure services are being rendered to children and families appropriately.
  • Provide support, training, and education to foster families and other relevant individuals on foster care policy and procedure.
  • Conduct investigations in accordance with state or agency regulations, as required.
  • Perform all other tasks as needed and/or assigned.
Requirements
  • Bachelor's or Master's degree in a human services area, such as social work, sociology, psychology, or a related field.
  • One year of experience as a social service worker, preferably in foster care or adoption.
  • Knowledge of agency policy, procedures, and operations, as well as state and federal social welfare laws and policies.
  • Ability to maintain records, prepare reports, conduct correspondence, communicate verbally with a range of clients and professionals, and maintain favorable public relations.
  • High level of communication skills, decision-making skills, and problem-solving techniques.
  • Ability to navigate computer systems comfortably.
  • Ability to work independently and as part of a team.
  • Valid Michigan Driver's License and ability to meet agency requirements for driving insurability, if applicable.
  • Ability to meet agency requirements for criminal background and central registry checks.
Preferred Qualifications
  • Knowledge of social work theory methods, principles, practices, techniques, and language for casework, group work, and community organization.
  • Experience working with diverse populations, including children and families from diverse cultural and socioeconomic backgrounds.
  • Knowledge of child abuse and neglect reporting requirements and procedures.
