We are seeking a highly skilled and compassionate Social Service Professional to join our team at Axesspointe Community Health Center. As a Foster Care Specialist, you will play a critical role in providing support and services to birth families, foster families, and adoptive families.
Key Responsibilities- Provide independent services to families, utilizing methods and techniques to complete processes and achieve desired outcomes.
- Develop and implement comprehensive assessments of individuals and families, identifying needs and providing appropriate services.
- Collaborate with agency staff, clients, and other professionals to ensure effective service delivery.
- Maintain accurate and confidential case records, adhering to agency policies and procedures.
- Participate in phone calls, home visits, court hearings, and other appointments to ensure services are rendered appropriately.
- Provide support, training, and education on foster care policy and procedure as required.
- Develop, implement, and monitor case plans, ensuring services are aligned with client needs.
- Prepare and maintain clear and concise reports and documents, adhering to agency and state licensing policies.
- Attend court hearings and provide accurate testimony and recommendations regarding case progress.
- Perform all other tasks as needed and/or assigned.
- Bachelor's or Master's degree in a human services area, such as social work, sociology, psychology, or family ecology.
- One year of experience as a social service worker, with completion of CWTI in foster care or adoption preferred.
- Knowledge of agency policy, procedures, and operations, as well as state and federal social welfare laws and policies.
- Ability to maintain records, prepare reports, conduct correspondence, and communicate verbally with clients and professionals.
- High level of communication, decision-making, and problem-solving skills.
- Ability to navigate computer systems comfortably and work independently.
- Good moral character, emotional stability, and professionalism.
- Valid Michigan Driver's License and ability to meet agency requirements for driving insurability, if applicable.
- Ability to meet agency requirements for criminal background and central registry checks.
Yearly Salary: $41,600 - $45,760
