We are seeking a highly skilled and compassionate Social Service Worker to join our team at Axesspointe Community Health Center. As a key member of our Foster Care & Adoption team, you will be responsible for providing critical support services to birth families, foster families, and adoptive families.
Key Responsibilities- Provide direct services to children, biological families, foster families, and adoptive families, including case management, counseling, and advocacy.
- Develop and implement comprehensive case plans to address the unique needs of each family.
- Collaborate with agency staff, community partners, and other stakeholders to ensure seamless service delivery.
- Maintain accurate and confidential case records, ensuring compliance with agency policies and regulatory requirements.
- Participate in phone calls, home visits, court hearings, and other appointments to ensure timely and effective service delivery.
- Provide training and education to foster families on foster care policy and procedure.
- Conduct comprehensive assessments of individuals and families, identifying areas of need and developing strategies to address them.
- Develop and implement service plans, ensuring alignment with agency goals and objectives.
- Prepare and maintain clear and concise reports and documents, including service plans, legal documents, and petitions.
- Attend court hearings and provide accurate testimony and recommendations regarding case progress and services rendered.
- Provide transportation assistance to clients as needed.
- Complete investigations in accordance with state or agency regulations, as required.
- Bachelor's or Master's degree in a human services field, such as social work, sociology, psychology, or a related field.
- One year of experience as a social service worker, preferably in foster care and adoption.
- Knowledge of agency policy, procedures, and operations, as well as state and federal social welfare laws and policies.
- Ability to maintain records, prepare reports, conduct correspondence, and communicate verbally with a range of clients and professionals.
- High level of communication skills, decision-making skills, and problem-solving techniques.
- Ability to navigate computer systems comfortably.
- Ability to work independently and as part of a team.
- Be of good moral character and demonstrate emotional stability and professionalism.
- Valid Michigan Driver's License and ability to meet agency requirements for driving insurability, if applicable.
- Ability to meet agency requirements for criminal background and central registry checks.
