Hospital Medicine Physician Assistant/Nurse Practitioner

We are seeking a highly skilled Physician Assistant/Nurse Practitioner to join our Hospital Medicine team at South Shore Health. As a key member of our team, you will be responsible for providing high-quality medical care to our patients, working closely with our physicians and other healthcare professionals to ensure seamless care transitions.
Key Responsibilities- Evaluate and diagnose patients, developing and implementing treatment plans in collaboration with our medical team.
- Order diagnostic tests, consultations, and procedures as necessary, ensuring timely and effective care.
- Manage chronic medical conditions, providing continuous care to hospitalized inpatients.
- Prescribe medications and write orders in accordance with current practice guidelines.
- Document patient care using electronic systems, maintaining accurate and comprehensive records.
- Collaborate with our patient care team to develop and facilitate care plans, ensuring timely and effective care.
- Proactively manage patients to prevent complications, providing urgent evaluation as needed.
- Prepare patients for discharge, including patient and family education, continuing care arrangements, and discharge prescriptions.
- Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution.
- Licensure and certification in good standing, including ACLS certification.
- Minimum 2 years of Acute Medicine experience, with a proven track record of delivering high-quality patient care.
