Job Summary

We are seeking a highly skilled and compassionate Community Residence Specialist to join our team at Loretto. As a key member of our residential care team, you will play a vital role in supporting the daily activities of older adults with psychiatric disabilities in our community homes.

Key Responsibilities
  • Develop and implement individual service plans with specific goals, tasks, and methods of measuring progress.
  • Provide guidance and support to residents in daily living skills, meal preparation, interpersonal and communication skills, medication management, crisis resolution, medical visits, transportation, and activities promoting independence.
  • Maintain contact with programs and service providers who work with residents.
  • Handle case management responsibilities for an assigned group of residents.
  • Complete service plans and utilization reviews for each resident and update as needed.
  • Responsible for medication management, including training staff, reviewing medication books, ordering refills, and preparing monthly medication sheets.
  • Develop a monthly menu with residents and counselors, prepare shopping lists, and purchase food within budget.
  • Pursue food stamps for eligible residents and complete re-certifications as needed.
  • Provide resident advocacy and linkages to ensure continuity with day programs, legal, medical, dental, psychiatric, and social services.
  • Complete or update other program/resident documentation, including progress notes, URs, discharge paperwork, etc.
  • Coordinate resident activity programs to enrich or improve resident lives.
  • Serve as a liaison between providers and residents to ensure Medicaid reinstatement of services for transportation.
  • Transport residents to and from day programs and/or appointments as needed.
  • Provide support and assistance in crisis/problematic situations as needed.
Requirements
  • Bachelor's Degree in a human service field, or Associate's Degree in a human service field and at least three years of experience in human services, or a High School Diploma and 5 years of experience in human services.
  • A valid and clean New York State Driver's License.
  • A positive customer-focused attitude.
  • Ability to develop a team.
  • Effective communication skills (verbal, written, listening).
  • Willingness to be flexible and perform extra duties as needed.
  • Ability to work effectively within a team and also independently.
  • Ability to multitask and meet deadlines.
  • Ability to work rotating shifts in a 24/7 environment and take call as needed.
  • Proficient with MS Office and able to learn new software rapidly.
