Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Yorktown Assisted Living Residence. As a LPN, you will play a vital role in providing high-quality care to our residents.

Responsibilities:
  • Provide safe and effective resident care, demonstrating knowledge and competency in completing assigned treatments and procedures.
  • Participate in the improvement process of quality resident care.
  • Incorporate age-specific needs, provision of care, procedures, and treatments.
  • Accurately complete routine tasks without the need for supervision.
  • Communicate pertinent resident information and plan of care before breaks, during transfer, and at shift report.
  • Assist residents with self-administration of medication as per physician orders and accurately document this in the medical record.
  • Directly supervise Home Health Aides and ensure that resident care is provided timely and in accordance with NYS regulations.
  • Complete reports/audits as assigned by Clinical Director and/or Administrator.
  • Assist the community with new resident admissions/readmissions.
  • Help residents and families acclimate to the community by providing guidance and emotional support.
Requirements:
  • Must possess a current New York State Licensed Practical Nurse (LPN) designation.
  • Requires knowledge and previous experience in med pass, use of electronic medical records (EMR), supervision of staff as well as geriatric care.
