Job Overview:

As a Client Support Coordinator, you will play a vital role in supporting individuals with varying ages, diagnoses, and needs in their homes. Your responsibilities will include implementing plans of care, administering medications, maintaining medication administration records, and documenting daily progress.

About the Position:

This is a challenging and rewarding position that requires strong communication and interpersonal skills. You will work independently and as part of a team to ensure the best possible outcomes for our clients. A typical day may involve assisting with personal care, activities of daily living (ADLs), and providing emotional support.

Key Responsibilities:

  • Implement plans of care and administer medications as ordered by physicians
  • Maintain accurate and up-to-date medication administration records
  • Document daily progress, including MAR, progress notes, and Innovation Waiver Documentation
  • Assist clients with ADLs, such as bathing, dressing, feeding, and toileting
  • Work at group homes during scheduled shifts, even if clients are absent
  • Perform other duties as assigned by management

Requirements:

To be successful in this role, you must have a strong ability to communicate effectively, work independently, and prioritize tasks. You should also possess basic computer skills and be able to maintain accurate records. A background in healthcare or social services is preferred but not required.

Salary: $38,000 - $48,000 per year
