Key Responsibilities

A HIV Care Coordination Manager plays a vital role at AHF by providing direction, support, and assistance in enhancing access to and retention of essential medical services to address and eliminate obstacles to HIV care. With a compassionate and knowledgeable approach, you will collaborate with our healthcare providers and center teams to ensure that every client receives the necessary support and access to available resources.

Your responsibilities will include:

  • Conducting Initial Assessments and creating Individualized Care Plans tailored to psychosocial needs and objectives.
  • Utilizing your expertise to address fundamental needs such as housing, nutrition, and clothing, while establishing connections with programs and agencies that cater to these needs.
  • Facilitating referrals, support, and assistance for clients lacking these services, thereby empowering them to better understand their health status and work towards achieving their healthcare goals.

As a HIV Care Coordination Manager, you will enhance AHF's reputation by leveraging your case management experience to meet clients where they are, identify strengths and opportunities for development, and assist them in setting realistic goals. Your collaborative and engaging skills will embody the essence of the Ryan White Care Act, serving as a safety net for uninsured HIV/AIDS patients within our most vulnerable populations.

Your assessments, care plans, referrals, and support will directly contribute to AHF's success in fulfilling the requirements of our Ryan White contract. Beyond contractual obligations, your efforts will align with the measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

The HIV Care Coordination Manager will be an integral part of a close-knit team, working alongside providers and clients to implement optimal care plans for non-medical needs, thereby improving access to care and retention. You will advocate for clients to minimize service gaps and eliminate barriers to service, guiding them through each step towards viral suppression, improved health outcomes, and enhanced quality of life.

Qualifications

Preferred qualifications include:

  • Associate or Bachelor’s degree in health or human services-related field.
  • A minimum of 2 years of experience working with individuals living with HIV may substitute for the bachelor’s degree.
  • High school diploma or General Education Development (GED) is required.
