Key Responsibilities

As a Healthcare Services Coordinator at AHF, you will leverage your knowledge and skills in the Ryan White Care Act Program to guarantee that our clients receive essential support and resources. By performing Needs Assessments, formulating Action/Care Plans, and educating patients, you will assist them in comprehending their health conditions and treatment options. Furthermore, you will collaborate with various programs and organizations to address psychosocial needs such as housing, nutrition, and clothing for patients who are in need of these services.

In this capacity, you will engage closely with clients to identify their strengths, establish objectives, and provide support for their overall health and well-being. Your contributions will play a vital role in AHF's ability to fulfill the requirements of our contracts and established benchmarks for HIV/AIDS care.

You will champion our key initiatives and ensure that AHF's mission and values inform your work on a daily basis.

To qualify for the position of Healthcare Services Coordinator, you should possess a Bachelor's or Master's degree in Social Science, Social Work, or Human Services. Previous experience in Case Management is preferred, and familiarity with Ryan White Services is advantageous.
