Job Summary

The Clinical Care Coordinator will be responsible for coordinating the clinical care of ECM Program clients. This includes identifying, organizing, coordinating, and monitoring services needed by the client. The ideal candidate will have a strong understanding of community and housing resources, as well as government benefits and the welfare system.

**Key Responsibilities:**

* Coordinate with clients, healthcare providers, and community resources to ensure seamless care
* Develop and implement care plans to meet the unique needs of each client
* Facilitate linkage to mental health, substance abuse, and medical services
* Provide transportation and accompaniment to office visits as needed
* Monitor treatment adherence and track interventions and outcomes
* Maintain accurate and up-to-date records of client interactions

**Requirements:**

* Minimum of two years' experience working with homeless and/or low and mixed-income populations
* Bachelor's Degree in Healthcare-related field preferred
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Valid driver's license and reliable transportation
