Premier Orthopaedics, a leading provider of orthopedic services, is seeking a highly skilled Patient Navigator to join our team at our corporate office in Newtown Square, PA.

Job Summary

The Patient Navigator will play a critical role in ensuring seamless patient care and navigation through our orthopedic services. This includes contacting patients prior to surgery to review their medical history, verifying clearance letters, and assessing their home and social situation to ensure they receive the necessary post-operative care.

Key Responsibilities
  • Contact patients prior to surgery to review medical history and verify clearance letters
  • Assess patients' home and social situation to ensure they receive necessary post-operative care
  • Review surgeon-specific education with patients regarding physical therapy, anticoagulation protocols, and patient engagement platforms
  • Call patients 1 day post-op to review their progress, address questions and concerns, and facilitate appointments as needed
  • Enter detailed documentation for ER visits, complications, and readmissions in the navigation platform
  • Return calls from voicemail and Athena messages from patients, hospitals, skilled nursing facilities, physical therapists, home care, and physician clinical staff
  • Generate and send weekly discharge disposition reports to facilities detailing patient plans and information
Requirements
  • Bachelor's of Science in Nursing (BSN) required
  • Minimum 5 years of experience in an orthopedic practice
  • Clinical and back-office experience preferred
  • Ability to follow up with patients and respond to inquiries promptly
