Job Summary

Mission Hospital is seeking a skilled and compassionate Manager for Case Management Services to join our team. As a key member of our healthcare team, you will be responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for your assigned area of responsibility.

Key Responsibilities
  • Oversee the day-to-day operations of the case management program, ensuring effective care coordination and case management practices are consistent with hospital policies and regulations.
  • Supervise and monitor professional and support staff, providing guidance and support to ensure high-quality patient care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans that promote optimal outcomes and resource utilization.
  • Monitor quality of interactions, documentation, and assessments of case management staff, ensuring compliance with hospital standards.
  • Coordinate activities that promote quality outcomes and patient throughput, while supporting a balance of optimal care and appropriate resource utilization.
Requirements
  • Associate Degree in Nursing or Nursing Diploma
  • 3+ years experience in clinical nursing
  • Current licensure as a Registered Nurse in the state(s) of practice according to law and regulation
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 2+ years supervisory experience
  • InterQual experience
