Director of Case Management and Social Services

We are seeking a highly skilled and experienced Director of Case Management and Social Services to join our team at Houston Methodist. As a key member of our leadership team, you will be responsible for overseeing the strategy, function, and operations of our Case Management and Social Services department.
Key Responsibilities- Develop and implement strategic plans to improve patient outcomes, reduce length of stay, and enhance the overall quality of care.
- Lead and manage a team of professionals to ensure the delivery of high-quality, patient-centered care.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and communication.
- Develop and maintain relationships with community providers, payers, and other stakeholders to ensure optimal resource utilization and patient outcomes.
- Monitor and analyze departmental performance metrics to identify areas for improvement and implement evidence-based solutions.
- Ensure compliance with regulatory requirements, accreditation standards, and organizational policies and procedures.
- Develop and implement education and training programs to enhance staff knowledge and skills.
- Participate in quality improvement initiatives and contribute to the development of departmental policies and procedures.
- Master's degree in a related field (e.g., social work, nursing, healthcare administration).
- At least 5 years of experience in case management and social services, with a minimum of 2 years in a leadership role.
- Strong leadership and management skills, with the ability to motivate and inspire a team.
- Excellent communication and interpersonal skills, with the ability to build and maintain relationships with diverse stakeholders.
- Ability to analyze complex data and develop evidence-based solutions.
- Knowledge of regulatory requirements, accreditation standards, and organizational policies and procedures.
