We are seeking a highly skilled and experienced Director of Case Management to lead our inpatient case management program. The successful candidate will be responsible for implementing and overseeing the program's components, including care facilitation, utilization management, case management, and discharge planning.
Key Responsibilities- Develop and implement systems and processes for care and utilization management, ensuring effective use of hospital resources.
- Oversee the collection, analysis, and reporting of financial and quality data related to utilization management, quality improvement, and performance improvement.
- Lead a high-performance team of case managers, promoting interdisciplinary collaboration and teamwork.
- Ensure safe care to patients, staff, and visitors, adhering to Memorial Hermann policies, procedures, and standards.
- Develop and implement performance improvement strategies, using data to drive decisions and improve outcomes.
- 5+ years of case management experience in an acute care hospital setting, with leadership and supervisory experience.
- Bachelor's degree in Nursing (BSN) or Master's degree in Social Work (MSW); Master's degree preferred.
- Current and valid license to practice as a Registered Nurse in the state of Texas or Master's Social Worker (LMSW) license.
- LCSW preferred and Certified Case Manager (CCM), Accredited Case Manager (ACM), or Fellowship of the American Academy of Case Management (FAACM) certification.
