Clinical Access Specialist Job Description

We are seeking a highly motivated Clinical Access Specialist to join our team at Tarzana Treatment Centers, Inc. As a Patient Navigator I, you will be responsible for completing preliminary screenings to determine the level of care and services required for recovery efforts. You will also conduct on-site and/or field-based outreach, engagement, re-engagement, and navigation services. This is an excellent opportunity to make a meaningful difference in the lives of others.

Responsibilities
  1. Initial navigation screenings to assess individual needs and service requirements.
  2. Enhanced patient navigation services to facilitate access to relevant programs and services.
  3. Maintenance of accurate and up-to-date records in the Access Log.

Qualifications
  • 1+ years of experience in case management.
  • Excellent interpersonal and communication skills.
  • Ability to work effectively with diverse populations.

Benefits and Compensation
  • $25.75 - $29.50 per hour (DOE).
