As a Discharge Planner RN, you will specialize in coordinating home care services and resources, acting as a vital link between our patients and the healthcare system. Your expertise will play a critical role in ensuring our patients receive the support they need to thrive at home.

Location: This position is part of our float team, serving Ann Arundel Medical Center, Suburban, and Sibley locations.

Your Responsibilities:

  • Bachelor's degree in nursing or a related field (BS preferred)
  • Licensure in clinical discipline (RN) in the State of Maryland required
  • Active CPR certification

Your Experience:

  • Minimum 2 years of experience in home care or discharge planning preferred
  • In-depth knowledge of Medicare Conditions of Participation and home care community resources
  • Thorough understanding of federal and state regulations, as well as third-party reimbursement guidelines for home care services

Compensation: Salary range: $34.00 - $50.92 per hour, commensurate with equity and experience for similar roles within the company.
