We offer a competitive salary range of $36.31 to $56.28 per hour, commensurate with experience and equity for roles of similar scope and responsibility.

Job Description

Your primary responsibility as a Discharge Planner RN will be to facilitate smooth discharges from our hospitals to home-based care. This involves conducting thorough assessments, developing personalized care plans, and coordinating services with our multidisciplinary teams.

Required Skills and Qualifications
  • A graduate of an accredited RN program, with a BSN preferred;
  • Active licensure as an RN in the State of Maryland; and
  • Current certification in CPR.
